before, you MUST still complete the process again for admission to this
program!

to mentor)
When you receive form, fill out and fax/mail to the office immediately!
You will be contacted by a representative from Hillcrest, who will schedule
an appointment with you to get your fingerprints done at the Hall of Justice & Records.
